I accept the terms of play, have read the rules and accept my liability stated here: By registering for this event each player claims to be covered by their own medical insurance for any illness or injury at the K13 Inc. Jonesboro Mud Ball Volleyball Tournament. Registrant is at least 14 years old. Registrant hereby waives and releases K13 Inc., Jonesboro Mud Ball and their affiliates and agents from any and all liability for any injuries or illness incurred while playing. Registrant hereby authorizes K13 Inc. and its agents to act for me according to their best judgment in any emergency requiring medical attention. Registrant will be responsible for any medical fees or other charges in connection withattendance and participation in the Volleyball Tournament. If registrant is under the age of 18, a parent or legal guardian must sign acknowledging that the above waiver also applies to their child.
